This form gives permission for Band Members to appear in photographs of Band performances/events.

Please Note: As we cannot prevent members of the public taking photographs of the Band, unfortuneately the only way to guarantee that your photograph is not taken, is not to take part.

DECLARATION

I understand that members of Lincoln District Scout Band may be photographed during band performances:
(a) By Band Officials (photographs to be used for promotional purposes - E.g. the Band Website),
(b) By members of the public watching the Band perform (these may be people unknown to us, therefore they cannot be CRB (Criminal Records Bureau) checked like a Warranted Scout Leader, nor can they be vouched for, in any way, by Lincoln District Scout Band, Lincoln District Scout Council or The Scout Association (UK)).

I give my permission for such photographs to be taken of:

To revoke this declaration (Parent or Guardian if under 18) please put your request in writing to the Bandmaster or any member of the Lincoln District Scout Band Management Committee.
